K-12 Student Accident

Claims management is provided by
Risk Strategies (Academic Health Plans).
There are three key documents Risk Strategies (Academic Health Plans) must have to process a claim:

1. Claim form
2. Fully Itemized Bill
3. Primary Insurance Explanation of Benefits

To file a claim, complete and sign the Claim Form in full at the time of injury or as quickly as possible after the injury (or submit  the online claim form).

The Basic Student Accident Insurance allows you to choose from two different plan options: Primary or Excess. These come with different deductible options.
