Basic Group Life Insurance (Hartford Life)
We are pleased that Seventh-day Adventist Church employers are providing a Group Life Insurance policy, (survivor benefit), to eligible employees in North America and Bermuda. Active full time employees, their spouses, and dependent children as defined in the Health Care Assistance Policy are insured under the plan.

Because this is an employer-provided benefit, there is no cost to you. Coverage under the Basic Life policy is as follows: Employee, $100,000; Spouse, $50,000; Dependent Children, $10,000; Stillborn, $750. No enrollment is necessary. However, you must complete a Beneficiary Form and place it on file with your employer. If you have not completed a Beneficiary Form or need to update your designations, please see your employer.

Supplemental Life Insurance (Hartford Life)
Though no one likes to think about life insurance, it may offer the financial protection that can secure your family’s future in the event that something should happen to you.

The Basic Life Insurance (survivor benefit) provided by your employer may not be enough to protect your family in their time of need. That’s why Supplemental Life Insurance is so important. Supplemental Life Insurance may help make sure you have the right amount of coverage for your family.

We are pleased to present a Supplemental Life Insurance product from Hartford Life at a very reasonable price. You are eligible to purchase this insurance at low group rates if you are an employee within the North American Division and work a minimum of 19 hours per week. The cost of your coverage will be deducted from your payroll on an after tax basis. You may purchase up to seven times your annual earnings, up to a maximum of $750,000; for spouse, the lesser of 100% of the employee amount or $250,000; for children, the lesser of 100% of the employee amount up to $25,000. Medical Underwriting will be required on coverage amounts that exceed pre-determined guarantee issue amounts.
