The North American Division (NAD) Health Care Assistance Plan (the Plan) is a “Church Plan” structured in accordance to Internal Revenue Service guidelines.
The Plan consists of four primary coverages; medical, dental, vision, and prescription drugs. Seventh-day Adventist Church employers sponsoring the Plan determine the eligibility status of their employees and dependents. Employee contributions vary by employer. The current NAD Health Care Assistance Plan became effective on July 1, 2006.
Consent for Release
HIPAA regulations prohibit the unauthorized disclosure of any individual's medical events to another individual. By completing and signing the Consent for Release Form individuals can authorize their Protected Health Information (PHI) to be discussed with the individuals of their choice.
